Applications and instructions for grants awarded by Four County Community Foundation are listed below. Please download the application forms and carefully review the information provided to ensure that your application package is complete. Especially note the deadline and number of copies that must be submitted. All applications must be received in the Four County Community Foundation office on or before the deadline date. General

Applications use a short form for requests under $1,000 and a long form for requests of $1,000 or more.

Healthly Seniors/Healthy Youth

Applications are for projects to promote health or provide treatment for health care for senior citizens and youth. Applications use a short form for requests under $1,000 and a long form for requests of $1,000 or more. Please review the Priorities below.

21st Century Education Fund Application

Application request up to $500.These grants are to provide classroom teachers with grants to benefit students. The grants are intended to fund projects in their entirety.

Kellogg Group Applications

Applications are for group grants which provide services to youth. Priority will be given to projects which address the challenges identified in their recent Needs Assessment Survey. Top priorities include: Youth Recreational Facilities, Drug Abuse, Alcohol Abuse, Harassment, Teen Violence, Getting a Job and Teen Pregnancy.
