Fund Representatives: Please use this form to submit grant requests from your fund.
NOTE: The request form is available in two formats:
- "Tab & Type" MS Word files. This option allows you to open the file in Microsoft Word, fill out the form on your computer, save it and print it out to mail to the Foundation.
- "Print & Write-In" PDF files, which require Adobe Acrobat Reader, available free online. For this option, you'll need to print the file first, fill it out by hand or typewriter, and then mail it to the Foundation.
